Homemodification

This wiki site allows health professionals, service providers, suppliers, patients and family caregivers to access resources for modifying homes in order to make the homes accessible for disabled residents. We plan to refer patients and service providers to this wiki site as soon as it is known that a patient is returning home and that the home requires modification to make it accessible.
